Assessment of occupational hazards and associated risks in fuzzy environment: A case study of a university chemical laboratory

Ozdemir, Yasal; Gul, Muhammet; Celik, Erkan

University laboratories involve various occupational hazards during the research and course activities, which might affect health and safety of both researchers and students, including chemical, physical, electrical, mechanical, as well as ergonomic and psychosocial risk factors. There are many aspects open to improvements in Occupational Health and Safety management of laboratories, and it is required to deal with risks, reduce them in an acceptable level, and implement control measures. In this study, a risk assessment approach is proposed for the university laboratory operations. The proposed approach incorporates 5S methodology, failure modes and effects analysis (FMEA), interval type-two fuzzy sets (IT2FSs), analytic hierarchy process (AHP), and the VlseKriterijumska Optimizacija I Kompromisno Resenje (VIKOR). AHP is injected into IT2FSs in the phase of evaluation and weighting of severity (S), occurrence (O), and detectability (D) parameters of FMEA. By applying IT2FVIKOR, it is aimed at prioritizing the emerged hazards in the chemical laboratory of the observed university. The proposed approach reveals the important hazards and suggests control measures for managing them. The study methodologically contributes to risk assessment in the knowledge, while a case study in a chemical laboratory of a newly established university offers an insight into education industry in safety improvement.

Heavy metal and arsenic concentrations in rainbow trout (Oncorhynchus mykiss) farmed in a dam reservoir on the Firat (Euphrates) River: Risk-based consumption advisories

Varol, Memet; Kaya, Gülderen Kurt; Alp, Alper

The concentrations of ten metals in rainbow trout (Oncorhynchus mykiss) farmed in the Karakaya Dam Reservoir (Turkey) on the Firat River were determined. The metal concentrations in rainbow trout did not exceed the maximum permissible levels. Biomagnification factors (BMF) of ten metals were < 1, indicating that these metals were not biomagnified. The estimated daily intake (EDI) of each metal was much lower than the respective tolerable daily intake (TDI). The target hazard quotient (THQ) for individual metal and total THQ for combined metals were lower than 1, indicating no health risk for consumers due to the intake of either individual metal or combined metals. The cancer risk (CR) value for toxic inorganic As was below 10–6, indicating no carcinogenic risk. According to the calculated maximum allowable consumption rates (CRmm), an adult may safely consume 28 meals of rainbow trout from the dam reservoir per month with no adverse carcinogenic and non-carcinogenic health effects. According to these results, the consumption of rainbow trout farmed in this dam reservoir is completely safe for human health.

Effects of role-playing scenarios on the self-efficacy of students in resisting against substance addiction: A pilot study

    Karatay, Gülnaz; Baş, Nazan Gürarslan

During the first phases of adolescent development, young people have little self-efficacy and resistance against substance use. The aim of this study was to demonstrate the effectiveness of role-playing scenarios on the self-efficacy of students in resisting substance use. A pre test and post test study design was used with a single group. The study was carried out with 245 secondary school students. The scenario-based training, developed by the researchers, was presented by the school counselors once a week for 4 weeks. For this purpose, a booklet of scenarios was prepared for the teachers. The role-playing scenarios were intended to improve adolescents’ abilities to say "no" to substance offers, to prevent them from becoming addicted to certain substances, and to call for help if needed. The data of the study were collected using the Personal Information Form and the Self-Efficacy for Adolescences Protecting Substance Abuse Scale. The obtained data were assessed using percentages, chi-square, t test, and F test in the SPSS software. Results showed that, after the training, the mean score in the Self-Efficacy for Adolescences Protecting Substance Abuse Scale increased significantly (103.20 ± 20.00) compared with before the training (92.11 ± 17.08) (P <.05). Short-term outcomes of the class-based scenario training were observed to be effective in the development of students’ self-efficacy to resist the temptations of substance use.

Pregnancy-Related Cardiovascular Complications: Case Presentation and Review of Literature

Altinsoy, Hasan Baki; Alatas, Ozkan; Colak, Salih; Atalay, Hakan; Dogan, Omer Faruk

Background: Cardiovascular complications that can cause severe catastrophic outcomes for both the mother and the fetus are rarely seen during pregnancy. Time to diagnosis is often delayed by a low degree of suspicion and atypical presentation. We report surgical strategies in three pregnant women with cardiovascular complications. Methods: A retrospective search from 2009 to 2016 identified three pregnant women who underwent urgent cardiac surgery. We used extracorporeal circulation (ECC) without cesarean section with careful follow-up of the fetuses during the perioperative and postoperative period. We used levosimendan as a potent inodilator in all patients to increase feto-placental blood flow and fetal heart rhythm. Results: Median time to diagnosis was 23.8 h (range 11.7-120 h) and median time from diagnosis to arrival in the operating theater was 9.8 h (range 7.4-19.8 h). One patient with prosthetic heart valve thrombosis underwent concomitant cesarean section prior to cardiac surgery. In a young pregnant woman who had spontaneous dissection of the left anterior descending artery, on-pump beating heart coronary artery bypass grafting was performed without cross clamping. Two and three months after surgery, cesarean sections were performed without any complication in two pregnant women. Conclusion: Because unusual cardiovascular complications are the main cause of maternal and/or fetus death during pregnancy, prompt and exact diagnosis is very important. Life-saving surgical strategy with the help of appropriate teams are necessary to optimize outcome for both mother and baby.

The effects of sebum configuration on Demodex spp. density

Demirdag, Hatice Gamze; Özcan, Hamdi; Gürsoy, Sule; Beker Akbulut, Gulcin

Background/aim: Demodex spp. are ectoparasites living in the pilosebaceous units, which feed on the host's sebum and cellular proteins. The protective barrier of the skin consists of sebum secretion, moisture, and the acid mantle. In this study, we aimed to determine the effects of skin sebum, moisture, pH levels, and sebum configuration on Demodex spp. density Materials and methods: Forty-five patients who had demodicosis were enrolled in the study group, while the control group consisted of 40 subjects without demodicosis. Body fat percentage, serum triglyceride and cholesterol levels, skin sebum, moisture, and pH levels were measured. Demodex spp. density was determined with a standardized skin surface biopsy. Sebum samples were taken from the forehead and a high-performance thin-layer chromatography (HPTLC) method was performed on these samples. Subsequently, densitometric analyses were applied to the HPTLC plates. Results: Demodex spp. were found on the cheeks and lived in an alkali environment. Skin sebum and moisture levels were low in all groups. The skin pH levels and cholesterol ester in the sebum configuration were determined to be significantly higher in the group with demodicosis. Conclusion: We suggest that Demodex spp. may use cholesterol ester in the sebum as nutriment. In other words, cholesterol ester may be a suitable growth medium for the proliferation of Demodex spp.

Ethnopharmacological survey of medicinal plants in Ulukisla (Nigde-Turkey)

Paksoy, Mehmet Yavuz; Selvi, Selami; Sauran, Ahmet

This study aimed to identify wild plants collected for medicinal purposes by the local people of Ulukisla County, located in the Central Anatolia Region of Turkey and to establish the uses and local names of these plants. A field study was carried out over a period of approximately 2 years (2002-2003). During this period, 59 vascular plant specimens were collected. Demographic characteristics of participants, local plant names, plant parts used and preparation methods of the plants were investigated and recorded. Within the scope of the study, the plant species were collected; herbarium materials were prepared; and the plant specimens were named. In addition, the relative importance value of the species was determined and was calculated for the medicinal plants included in the study. A total of 59 medical plants belonging to 27 families were identified in the region. The most common families were Lamiaceae, Rosaceae, Asteraceae, Poaceae and Apiaceae. This study identified not only the wild plants collected for medical purposes by local people of Ulukisla County in the Central Anatolia Region, but also the uses and local names of these plants. Comparison of the data obtained in this study from the plants growing in Ulukisla with the experimental data obtained in previous laboratory studies showed mostly the same ethnobotanical usages. The plant flora of Ulukisla is threatened by such factors as grazing, expansion of new agricultural lands, and unsustainable picking of plants to generate income. Steps should be taken immediately to ensure the inclusion of relevant flora within conservation designations. Published by Elsevier GmbH.

Cultural perceptions and clinical experiences of nursing students in Eastern Turkey

    Karatay, G.; Bowers, B.; Karadag, E. B.; Demir, M. C.

Aim: This study explored Turkish nursing students' perceptions of providing care to patients culturally different from themselves. Background: Increasing migration will increase the need for nurses to provide care across cultural groups. Methods: Twenty one students in the second year of a 4-year nursing programme participated in 3 focus groups. Data were analysed using directed content analysis. Research questions were based on Campinha-Bacote's model. Results: Three themes were identified: perceived cultural barriers, perceived cultural facilitators and identifying culturally sensitive actions. Generally, students were able to define culture but were unable to say how culture would affect nursing practice. Discussion: Students were unprepared to practice in a multicultural setting. Cultural awareness is insufficient for determining how to respond to cultural differences. Limitations: The study is limited by its restriction to a single school of nursing and a single curriculum. Conclusions: The multiple, ongoing political, religious and ethnic conflicts will require nurses to provide care to patients from other cultural groups, in some instances to people identified as adversaries to the group the nurse may represent. Understanding cultural differences is insufficient to do this effectively. Implications for nursing education: Learning culturally competent care requires opportunities to provide, be guided through and reflect on care to individuals from different cultural groups. Implications for organizational and public policies: Standards for culturally competent care should be adopted by all care delivery settings. Public and organizational policies openly declaring healthcare settings as cultural safe zones, and explicit organizational commitment to culturally safe care, would set clear expectations for providers and help ensure a positive patient experience.

Chronic exposure of tilapia (Oreochromis niloticus) to iron oxide nanoparticles: Effects of particle morphology on accumulation, elimination, hematology and immune responses

Ates, Mehmet; Demir, Veysel; Arslan, Zikri; Kaya, Hasan; Yilmaz, Sevdan; Camas, Mustafa

Effects of chronic exposure to alpha and gamma iron oxide nanoparticles (alpha-Fe2O3 and gamma-Fe2O3 NPs) were investigated through exposure of tilapia (Oreochromis niloticus) to 0.1, 0.5 and 1.0 mg/L (9.2 x 10(-4), 4.6 x 10(-3) and 9.2 x 10(-3) mM) aqueous suspensions for 60 days. Fish were then transferred to NP-free freshwater and allowed to eliminate ingested NPs for 30 days. The organs, including gills, liver, kidney, intestine, brain, spleen, and muscle tissue of the fish were analyzed to determine the accumulation, physiological distribution and elimination of the Fe2O3 NPs. Largest accumulation occurred in spleen followed by intestine, kidney, liver, gills, brain and muscle tissue. Fish exposed to gamma-Fe2O3 NPs possessed significantly higher Fe in all organs. Accumulation in spleen was fast and independent of NP concentration reaching to maximum levels by the end of the first sampling period (30th day). Dissolved Fe levels in water were very negligible ranging at 4-6 mu g/L for alpha-Fe2O3 and 17-21 mu g/L for gamma-Fe2O3 NPs (for 1 mg/L suspensions). Despite that, Fe levels in gills and brain reflect more dissolved Fe accumulation from metastable gamma-Fe2O3 polymorph. Ingested NPs cleared from the organs completely within 30-day elimination period, except the liver and spleen. Liver contained about 31% of alpha- and 46% of gamma-Fe2O3, while spleen retained about 62% of alpha- and 35% of the gamma-polymorph. No significant disturbances were observed in hematological parameters, including hemoglobin, hematocrit, red blood cell and white blood cell counts (p > 0.05). Serum glucose (GLU) levels decreased in treatments exposed to 1.0 mg/L of gamma-Fe2O3 NPs at day 30 (p < 0.05). In contrast, GLU levels increased during the elimination period for 1.0 mg/L alpha-Fe2O3 NPs treatments (p < 0.05). Transient increases occurred in glutamic oxaloacetic transaminase (GOT), glutamic pyruvic transaminase (GPT), and lactate dehydrogenase (LDH). Serum Fe levels did not change during exposure (p > 0.05), but increased significantly within elimination period due to mobilization of ingested NPs from liver and spleen to blood. Though respiratory burst activity was not affected (p > 0.05), lysozyme activity (LA) was suppressed suggesting an immunosuppressive effects from both Fe2O3 NPs (p < 0.05). In contrast, myeloperoxidase (MPO) levels increased significantly in treatments exposed to alpha-Fe2O3 NPs (p < 0.05), and the effect from gamma-polymorph was marginal (p > 0.05). The results indicate that morphological differences of Fe2O3 NPs could induce differential uptake, assimilation and immunotoxic effects on O. niloticus under chronic exposure. (C) 2016 Elsevier B.V. The in vitro effect of cypermethrin on quality and oxidative stress indices of rainbow trout Oncorhynchus mykiss spermatozoa

Kutluyer, Filiz; Benzer, Fulya; Erisir, Mine; Ögretmen, Fatih; Inanan, Burak Evren

There is limited information on the scientific literature about the effect of in vitro exposure of fish sperm to pesticides. In vitro effect of cypermethrin on sperm quality and oxidative stress has not yet been fully investigated. Therefore, the effects of cypermethrin, a type II pyrethroid insecticide, on quality and oxidative stress of spermatozoa were examined in vitro. To explore the potential in vitro toxicity of cypermethrin, fish spermatozoa were incubated with different concentrations of cypermethrin (1.025, 2.05 and 4.1 mu g/l) for 2 h. The motility rate and duration of sperm were determined after exposure to cypermethrin. Reduced glutathione (GSH), glutathione peroxidase (GSH-Px), catalase (CAT) and malondialdehyde (MDA) in spermatozoa were analyzed for determination of oxidant and antioxidant balance. Our results indicated that spermatozoa motility and duration significantly decreased with exposure to cypermethrin. Additionally, activity of GSH-Px (P < 0.05) and MDA and GSH levels increased in a concentration-dependent manner while CAT activity decreased (P < 0.05). Consequently, the oxidant and antioxidant status and sperm quality were affected by quantitative changes and different concentrations of cypermethrin. (C) 2015 Elsevier Inc. All rights reserved.

An artificial neural network-based earthquake casualty estimation model for Istanbul city

    Gul, Muhammet; Guneri, Ali Fuat

Earthquakes over the world cause serious damages for people and the economy. The establishment of an appropriate model to estimate potential losses or injuries in earthquake disasters is fundamental to decrease their impacts and losses and effectively respond and mitigate. Turkey as a country that experienced many earthquakes in the last century and had serious human and financial losses needs a comprehensive knowledge of consequences of devastating earthquakes to be able to plan for the future. Artificial neural networks (ANNs) have abilities to solve and analyse complex relations as an appropriate method to estimate number of injuries. In this study, an ANN model is built up for the earthquake casualty prediction, which takes earthquake occurrence time, earthquake magnitude, and population density as the predictors and employs 21 Mw > 5 earthquake disasters occurred in Turkey from 1975 as samples for the training of the network. The model was then tested on a study region consisting of four districts in Istanbul which is estimated to have the highest injury rate according to the earlier reports and generated estimations of the expected number of injured people. Results show that 99.9 % of the variability in the number of injured people is predictable with using this ANN model. Comparison of actual values and estimated output values in the ANN model was also found apparently very close to each other. According to the test case study results, when the value of earthquake magnitude is 6.5 Mw, number of injured people has increased in a sharp trend compared to the previous magnitude value (6 Mw) for all districts. Estimated number of injured people in daytime is obtained higher than at night when the earthquake magnitude is 5 and 5.5 Mw. The highest value in estimated number of injured people has emerged in Fatih district as 7241 for a 7.5 Mw daytime earthquake. On conclusion, it was deduced that the model can reveal accurate estimation of casualties and it can provide information in order to develop mitigation policies, especially in earthquake emergency service management.

Önal, Yunus; Depci, Tolga; Önal, Ayten; Önal, Ali; Üner, Tijen

Asbestos is dangerous for health and is forbidden to use in Turkey, but villagers who live in rural eastern part still use it especially for their home for insulation purposes and even as a substitute for baby powder. Therefore, geological and chemical properties of this type of soil, which is near the settlement, should be characterized and identified from the point of view of health. Beypinari asbestos deposits are located at Beypinari village, about 90 km southeast of Sivas, Turkey within Central Anatolian Ophiolites. Therefore, in the present study, the existence of chrysotile in Beypinari asbestos deposit were characterized and identified by mineralogical analysis and by major, trace, REE analysis. The results show that the samples contain different amount of calcite mineral, the main mineral being chrysotile.

Emergency department performance evaluation by an integrated simulation and interval type-2 fuzzy MCDM-based scenario analysis

Gul, Muhammet; Celik, Erkan; Gumus, Alev Taskin; Guneri, Ali Fuat

Emergency departments (EDs) have an important position in healthcare system environment because of their serviceability to the patients with several illnesses and injuries at all hours of the day and night. Therefore, efficient management of these departments by executives and service providers is only possible with effective tools such as simulation. The concept of multiple criteria decision-making (MCDM) is applicable by computer simulation for decision analysis and performance evaluation of an ED. This study proposes an integration of computer simulation, MCDM methods of interval type-2 fuzzy analytic hierarchy process (IT2FAHP), and ELECTRE for an ED system in a university hospital. Due to the high uncertainty in ED operations, interval type-2 fuzzy sets (IT2FSs) are used in MCDM process. Consequently, the integrated method can be used as a useful tool to evaluate the ED performance and select the best scenario considering different numbers of nurses and doctors for three shifts.

Rutin attenuates gentamicin-induced renal damage by reducing oxidative stress, inflammation, apoptosis, and autophagy in rats

Kandemir, Fatih Mehmet; Ozkaraca, Mustafa; Yildirim, Betul Apaydin; Hanedan, Basak; Kirbas, Akin; Kilic, Kubra; Aktas, Esra; Benzer, Fulya

Gentamicin is commonly used against gram-negative microorganisms. Its therapeutic use is mainly limited by nephrotoxicity. This study was aimed at evaluating the effect of rutin on oxidative stress, inflammation, apoptosis, and autophagy in gentamicin-induced nephrotoxicity in rats. The rats were treated with saline intraperitoneally (group I), 150 mg/kg of rutin orally (group II), 80 mg/kg of gentamicin intraperitoneally for 8 d (group III), or 150 mg/kg of rutin plus 80 mg/kg of gentamicin (group IV). The serum urea, creatinine, kidney malondialdehyde (MDA), and reduced glutathione (GSH) levels and superoxide dismutase (SOD), catalase (CAT), and glutathione peroxidase (GPx) activity and protein concentration were measured, and renal histopathology analysis and immunohistochemical staining were performed. Rutin pretreatment attenuated nephrotoxicity induced by gentamicin by reducing the urea, creatinine, and MDA levels and increasing the SOD, CAT, and GPx activity, and the GSH levels. The rutin also inhibited inducible nitric oxide synthase (iNOS), cleaved caspase-3 and light chain 3B (LC3B), as evidenced by immunohistochemical staining. The present study demonstrates that rutin exhibits antioxidant, anti-inflammatory, anti-apoptotic, and anti-autophagic effects and that it attenuates gentamicin-induced nephrotoxicity in rats.
